Role: Hospital Social Work Team Manager Closing Date: 7th December 2025 In Blackburn with Darwen we believe we have the solid foundations for providing excellent social work services to local people. We are looking for an enthusiastic and committed Hospital Team Manager to lead the Hospital Discharge Team. You will be responsible for promoting Blackburn with Darwen’s vision, objectives and priorities effectively to our staff, partners and the public. You will have excellent communication and organisational skills alongside ensuring that the residents of Blackburn with Darwen are discharged from hospital in a safe and timely manner. This role will be an integral part of the management team working collaboratively with colleagues across the local authority departments and with our health partners leading the vision, transformation and strategic direction of the Council. As a Team Manager you will provide leadership for the Intermediate Tier service in Blackburn with Darwen; with the overarching aims: Able to demonstrate the high level of knowledge and skills required to: In your position as the Hospital Team Manager you will offer leadership to the local authority social workers and assessment and review officers. Providing professional oversight, supervision, advice and support in the management of the team. You will understand the value and importance of integrated working with partner agencies in delivering support and protection to individuals, carers and their families whilst ensuring safe and effective discharges from hospital.
