In this rewarding role, you will support vulnerable people to maintain and regain their independence, enabling them to live safely and comfortably in their own homes. Using a strengths-based approach, you will help individuals improve their quality of life and achieve their personal goals. You will also play a key part in supporting people as they transition home from hospital, working within Discharge to Assess pathways, as well as providing complex and palliative care support where needed. Previous experience in care is beneficial but not essential—we welcome applicants with the right values and a willingness to learn. This role involves working on a rota basis, including weekends, bank holidays, and shift patterns. As a front line care provider we ensure Government guidance is implemented and practiced at all times with regards to the current climate.
