We are seeking an experienced and motivated social work professional to join our Adult Social Care Management Team as the Preparing for Adulthood and Transitions Operational Lead.

This is a key leadership opportunity in adult social care to shape and drive high quality transitions for young people moving into adulthood. You will ensure early, person-centred planning from Year 9, working alongside Children’s Services, Education, Health, and partner agencies to deliver seamless, outcome focused transitions.


Key Responsibilities

  • Provide operational oversight of the transitions pathway, ensuring timely and lawful Care Act assessments, reviews, and safeguarding.
  • Lead on quality assurance of practice, ensuring robust and defensible decision-making across Care Act and Mental Capacity Act processes.
  • Drive performance, manage demand, and ensure safe allocation and oversight of caseloads.
  • Promote co-production with young people and families, supporting independence and positive life outcomes.
  • Strengthen multi-agency working and represent Adult Social Care at strategic and operational forums.
  • Support workforce development through reflective supervision, guidance, and practice leadership.

You will be a qualified Social Worker registered with Social Work England, with:

  • Significant experience in adult social care and transitions work
  • Strong knowledge of Care Act, SEND, safeguarding, and MCA/DoLS
  • Proven leadership, quality assurance, and performance management skills
  • Experience of working across complex multi-agency environments
  • A passion for improving outcomes for young people and families
