Contract Type: Permanent Hours: 37 per week Salary: NJC Grade F SCP 19-24 £32,061 - £35,412 per annum Closing Date: Wednesday 26th August 2026 at 23:59 Home Care and Reablement Service Following the success of the development of this role within the Home Care and Reablement Service, an exciting opportunity has arisen to expand our existing workforce. We are seeking an enthusiastic, motivated and experienced Care and Support Worker to join our dedicated team and play a key role in supporting people to live independently within their own homes and communities. This is a rewarding opportunity to contribute to the assessment, coordination and delivery of care and support that promotes independence, wellbeing and positive outcomes. Working within a strengths-based, reablement-focused and proportionate care approach, you will support individuals to maximise their abilities, regain confidence and reduce reliance on longer-term services wherever appropriate. As a Care and Support Worker, you will undertake first visits, complete risk assessments, develop person-centred care and support plans, and establish medication care plans to enable the safe delivery of care across the community. You will provide both planned and unplanned support, helping the service respond effectively to changing needs and emergency situations. A key aspect of the role is supporting individuals through Discharge to Assess pathways, enabling timely hospital discharges and helping people recover safely and independently at home. You will also contribute to reviews of care and support, ensuring services remain outcome-focused, proportionate and responsive to changing needs. The role includes responsibilities as part of a Trusted Assessor (Level 3) function, including the assessment, provision and fitting of low-level equipment to support independence and reduce risks within the home environment.
