Job Title:       Resource Planner

Team:             Home Support

Hours:            28 per week

Grade:            D SCP 6  £23,893 to SCP 11 £25,979 + 7% enhancement weekend working

                        (Pro rota salary based on 28hrs is £18,081)

Contract:        1 x Permanent and 1 Fixed Term for 6 months.

What do we do?

The service provides a stepping stone to independence, by supporting people to regain lost skills, learn new skills, and adapt to the challenges that living independently can present.

You will work within a CQC “Good” rated - Home Support Reablement Council Team, and will have an opportunity to work with other professionals such as our colleagues in the NHS, Social Workers, Occupational Therapists etc to provide all aspects of care.

Home Support Reablement is a short-term service, meaning there is a quick turn over of service users.

What is the role?

The role of a Resource Planner is to:

• inputting referrals onto the system

• inputting packages of support onto the system

• communicate using email and telephone

• responding to emergency referrals

• increasing and decreasing packages of support

• deploying staff in the most effective manner

What will your working week look like?

• 28-hour contracts available

• Rolling 3-week rota

• Variety of early, lates and weekend shifts

Working between 7am – 10pm

About You

Do you have…

• experience using IT systems?

• a passion to support and care for adults in the community?

• the ability to work on your own initiative?

• the ability to communicate clearly with people of all levels?

A comprehensive induction and training programme will be provided on succession into the role.

You must ensure that the service is delivered to the highest achievable professional standards to support vulnerable adults with both rehabilitative and / or mental health needs to retain community living.

Your work will be compliant with all CQC regulations, and Bolton Council policies and procedures.
