Job Description

The Older Adult Community Mental Health Team is looking to recruit a social worker who has experience of working with older adults and or older adults with Mental Illness.  Prior experience of working within the CPA framework and Recovery model is desirable.  The post holder will work as part of a multi-disciplinary team covering the Bolton area.

Full induction will be provided to the post holder.

About You

We are seeking to recruit a specialist social worker who will, in conjunction with multidisciplinary team members from Greater Manchester Mental Health NHS Foundation Trust, provide advice and consultation, assessment and support to carers, staff, voluntary and independent sector organisations who provide care to older adults with organic and functional mental disorders in Bolton. 

The post holder will be a member of a M.D.T providing expert advice to promote independence and ensure appropriate provision of treatment and care.

The post holder will work within the CPA process and Recovery model in addition to the Council’s personalisation agenda with a particular focus on supporting the timely assessment and discharge planning from the older adult’s mental health in-patient wards.

The service operates 365 days per year therefore the post holder will be required to work weekends and bank holidays on a rota basis.

AMHP qualification is desirable but not essential. The successful candidate must be willing to undertake AMHP training.
