Your role:

You will work with people, their families, carers and service providers from all sectors of the community, health and social care organisations.

You will assist people to have increased choice and control in the way that they organise their social care support. You will carry out proportionate social care assessments and reviews and support people to create their individualised care and support plan.

You will access support from both informal (family and friends) and formal (private, voluntary sector and council) groups and work with professionals and partners to clarify the individual’s assessment of need.

About you:

The ideal person will be an excellent communicator with strong interpersonal and customercare skills, able to engage, negotiate and appropriately challenge others. They will understand relevant social care legislation and safeguarding procedures, work well independently or in a team, and manage a varied caseload effectively. Strong numeracy, literacy and analytical skills are essential, along with the ability to use initiative and apply creative, flexible problemsolving approaches.

The Adult Social Care Hospital Discharge Team is a multidisciplinary team consisting of Health and Social Care colleagues. Your role will be to ensure safe, timely, and appropriate transition for patients from hospital to their community or another care setting, by conducting person-centred assessments, developing care plans, carrying out face to face assessments, collaborating with multidisciplinary teams, and connecting individuals with community resources.
